1. Acknowledgment of Risk
I, the Participant, acknowledge that I am voluntarily participating in Pilates classes provided by FOUR Pilates, which may include Mat and Reformer Pilates sessions at my home or home gym. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by me as a result of my participation in these activities.

5. Waiver of Liability
I, on behalf of myself, my heirs, personal representatives, and assigns, hereby release, waive, discharge, and covenant not to sue FOUR Pilates, its owner, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me while participating in Pilates classes, or while on the premises where the classes are conducted.

6. Indemnification
I agree to indemnify and hold harmless FOUR Pilates from any loss, liability, damage, or costs, including court costs and attorney’s fees, that they may incur due to my participation in said activities, whether caused by negligence or otherwise.
